Job Overview

The Pharmacy Medication Transition Technician assists inpatient and outpatient pharmacists with medication history, reconciliation, to ensure that any differences between medications taken before admission and medications ordered upon admission for a patient are intentional and not accidental. Works to promote an efficient discharge prescription process in collaboration with the outpatient pharmacy.

Additional Information

Department: Outpatient Pharmacy Hours per week: 40, eligible for benefits Shift: Day/Evening. Monday - Friday, 8am-8pm. Saturday-Sunday, 9am-2pm. Shift length varies between 8-10 hours. Requires rotating weekends, but no holidays at this time.

Qualifications

Education: High School Diploma or Equivalent Experience: Two (2) years of Pharmacy Technician Experience. Medication Reconciliation Technician or Meds to Beds experience is preferred. Equivalency: Completion of pharmacy technician program may substitute for one (1) year of the experience requirement. OR Completion of pharmacy technician program plus one (1) year of experience performing medication prior authorizations may substitute for two (2) years of pharmacy technician experience. Certification(s): State of Colorado Pharmacy Technician (PHAT) Certification Certificate through a nationally recognized certification board or body approved by the National Association of Boards of Pharmacy (NABP) such as the Pharmacy Technician Certification Board (PTCB) or National Healthcare Association (NHA) Medication History Certificate within 12 months of hire

Adheres to the general ho pital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships, and team building.
