Community Health Access Coalition Healthcare Services Coordinator in Sault Ste Marie, Michigan
The Healthcare Services Coordinator is responsible for (1) managing the access to healthcare program for all residents in the service area under the guidance of the Executive Director (2) serve as a liaison for physicians, hospitals, and other individuals and institutions in the assigned area (3) assure that the needs of CHAC clients are addressed compassionately, effectively, and with dignity. Duties and Responsibilities: Facilitate program enrollment and community outreach/education Serve as the liaison with healthcare partners in service area Maintain patient information systems Participate in community workgroups Facilitate fundraising activities throughout service area Work as part of the CHAC team to cover other positions when needed Perform other related duties as assigned
